Electronic Medicines Optimisation Pathway (EMOP) is reducing medication errors when patients are discharged from hospital by providing community pharmacists with updated medication information.
The tool, also referred to as transfers of care around medicines (TCAM), is a national AHSN Network adoption and spread initiative.
